It also proposes to establish baseline information to help clinicians determine or predict power wheelchair driving performance and to develop service plans to address those driving skills that need improvement or compensation. Sixty-two adult power wheelchairs users were recruited. Standardized instruments were used to evaluate visual perceptual skills, visual function, cognitive skills, and personality traits. The results of these evaluations were then correlated with participants’ scores on a power wheelchair performance test. Strong correlations were found between power wheelchair driving performance and visual perception (p = .000), ocular motor function (p = .000 and p < or = .001), stereodepth perception (p < or = .001), and alertness to the environment (p < or = .001). No significant correlations were found between personality traits and power wheelchair driving performance. These results indicate that good visual perceptual skills, visual function, and various aspects of cognition are necessary for proficient power wheelchair use. These data will assist clinicians in identifying significant factors to consider when evaluating and training clients for power wheelchair use.

The purpose of this study was to examine the effect of handrim size (0.54, 0.43, and 0.32 m) on mechanical energy and power flow during wheelchair propulsion. METHODS: Twelve young normal male adults (mean age 23.5 years old) were recruited in this study. Both 3-D kinematic and kinetic data of the upper extremity were collected synchronously using a Hi-Res Expert Vision motion system and an instrumented wheel during wheelchair propulsion. FINDINGS: The kinetic, potential and total mechanical energy of the upper extremity increased as the handrim size increased. For each upper arm segment, the joint translational power and the rotational power of the proximal joint increased with increasing handrim size. The work done during a complete propulsion cycle with the larger handrim size is significantly larger than that using a smaller handrim (P<0.05). INTERPRETATION: The increased kinetic, potential and total mechanical energy were due to the increased linear velocity and the elevated positions of the upper extremity segments. The shoulder and trunk flexors increased the magnitude of their concentric contractions during propulsion with the large handrim as increased output power is required. By using mechanical energy and power flow analysis techniques, we evaluated the previously-reported effect of handrim size on mechanical cost and provided insight into the relationship between the two.

INTERVENTIONS: Participants propelled both their own manual wheelchairs and a PAPAW 3 times over an ADL course. The order in which the 2 different wheelchairs were presented to the participants was randomized. MAIN OUTCOME MEASURES: Each participant’s heart rate was monitored throughout testing by a digital, wireless heart-rate monitor. Time to complete the course was recorded, and participants were surveyed with a visual analog scale after the first, third, fourth, and sixth trials to determine the ease of completing each obstacle and their ergonomic preferences between the 2 wheelchairs. Participants also were observed throughout the trials to determine how much assistance they needed to complete each obstacle course. RESULTS: After using a Bonferroni adjustment, 4 obstacles (carpet, dimple strips, up a ramp, up a curb cut) were rated as being significantly easier ( P <.001) to complete when using the PAPAW. Participants also showed a significant decrease in mean heart rate throughout all 3 trials ( P =.015, P =.001, P =.003, respectively) when using a PAPAW. The amount of assistance needed by participants, the responses to ergonomic questions, and the overall time to complete the ADL course did not differ significantly between the 2 power wheelchairs. CONCLUSIONS: For subjects with tetraplegia, PAPAWs have the potential to improve functional capabilities during certain ADLs, especially when propelling up ramps, over uneven surfaces, and over thick carpet.

The purpose of this study was to compare submaximal physiological responses and indices of mechanical efficiency between asynchronous and synchronous arm ergometry. Thirteen medicare power wheelchairs -dependent trained athletes performed eight steady-state incremental bouts of exercise (0 to 140 W), each lasting 4 min, using synchronous and asynchronous arm-cranking strategies. Physiological measures included oxygen uptake (VO2), heart rate, and blood lactate concentration. The power wheelchairs outputs corresponding to fixed whole blood lactate concentrations of 2.0 to 4.0 mmol x l(-1) were calculated using linear interpolation. Mechanical efficiency indices - gross efficiency, net efficiency, and work efficiency - were also calculated. An analysis of variance with repeated measures was applied to determine the effect of crank mode on the physiological parameters. Oxygen uptake was on average 10% lower (P < 0.01), and both net efficiency (P < 0.01) and gross efficiency (P < 0.01) were higher, during the asynchronous strategy at both 60 and 80 W (gross efficiency: 16.9 +/- 2.0% vs. 14.7 +/- 2.4% and 17.5 +/- 1.8% vs. 15.9 +/- 2.6% at 60 and 80 W respectively). There were no differences in heart rate, blood lactate concentration or power output at either of the blood lactate reference points between the asynchronous and synchronous strategies (P > 0.05). In conclusion, test specificity is an important consideration. If a synchronous strategy is to be adopted, it is likely to result in lower efficiency than an asynchronous strategy. The RPE data supported this finding, with a tendency for lower ratings with the 180 mm crank (9 +/- 2 vs. 10 +/- 3). The short crank length when used at 85 rev min(-1) was found to be the most efficient (GE 21.4 +/- 3.1%). In conclusion, crank length has a significant effect on ME in hand cycling. Unilateral neglect is a challenging disorder that pervades a range of behaviours following stroke and hampers recovery. Although a preponderance of clinical studies measure performance on a range of bedside assessments, including line bisection and medicare power wheelchairs, handicap scooters cancellation tasks, there have been calls for studies to embrace more relevant functional measures. Here, for the first time, we present data from two separate tasks that characterise the performance of seven patients with unilateral neglect when navigating a power wheelchair. The tasks involved negotiating an obstacle course and steering a central path between gaps of different sizes. Results from the obstacle course confirmed the clinical observation and predicted bias of contralesional errors. However, the second task revealed a robust “crossover” effect. Patients deviated to the ipsilesional side for large gaps but deviated increasingly contralesionally when steering through small gaps in behaviour that was analogous to that previously shown on line bisection tasks. Contrary to being seen as an unintuitive finding, further analysis of these errors suggests that patients are giving disproportionate weight to the location of the ipsilesional object when plotting a midline course between two objects. Our results provide a platform for further studies to investigate the modulation and rehabilitation of this important skill.

OBJECTIVE: To investigate exercise-induced, metabolic, pulmonary, cardiovascular and energetic reactions of a paraplegic athlete during a city marathon. SETTING: City marathon Cologne, Germany. SUBJECT: We tested a 27-year-old male athlete from the German national team. The handicap scooters of the athlete was a complete traumatic spinal cord injury ASIA/IMSOP Grade A at the level of the fourth thoratic vertebra (Th4). METHOD: In the competition, the athlete used his own race handbike whose crank system had been exchanged for a power measurement system with registering option. For measuring ventilatory gas parameters during exercise, a portable spirometric system was used. The athlete managed to finish the marathon race in 1:48:54 h. RESULTS: The mean oxygen uptake (VO2) during the marathon was 1580 ml/min, with a maximum value of 2535 ml/min. The mean heart frequency was 137 bpm with a maximum of 157 bpm. During the race the mean energy consumption was 463 kcal/h with a maximum of 758 kcal/h. Prior to the race, the blood lactate value was 2.9 mmol/l; after 10 km 4.4 mmol/l; after 20 km 2.9 mmol/l, and after 30 km 2.9 mmol/l. CONCLUSION: Competition-oriented medical scooters should concentrate on exercise units of long duration at low intensities–like marathon runners or cyclists–in order to improve their aerobic performance capacity. However, it has to be pointed out that paraplegic athletes develop relatively high metabolic intensities in competition and that the variability of their physiological parameters is considerably high.

PURPOSE: This qualitative study investigated the lived experience of senior stroke survivors who used prescribed medicare power wheelchairs in their home and community. METHODS: The study involved semi-structured, in-depth interviews that were conducted with 10 participants, aged 70 to 80, who had used a power wheelchairs for a mean of 5.6 years. All participants used manual wheelchairs and two additionally used a power wheelchair and a scooter. RESULTS: A constant comparative inductive method of analysis of the data yielded several themes which are examined within the International Classification of Functioning, Disability and Health framework of body functions and structure, activity limitations, participation, and contextual factors. Overall, the wheelchair, manual or power, was an enabler of community participation among the participants. However, the wheelchair’s use also created problems, such as limiting destinations and creating increased dependence on others. PRACTICE IMPLICATIONS: A balance scale model was proposed to facilitate an understanding of how the factors affecting community participation interact in this population. This model can assist occupational therapists who prescribe wheelchairs for stroke clients for use in the community.

Acupuncture for chronic shoulder pain in persons with spinal cord injury: a small-scale clinical trial
OBJECTIVE: To determine the efficacy of acupuncture in the treatment of chronic musculoskeletal shoulder pain in subjects with spinal cord injury (SCI). DESIGN: Randomized, double blind (participants, evaluator), placebo (invasive sham) controlled trial. SETTING: Clinical research center. PARTICIPANTS: Seventeen manual wheelchair-using subjects with chronic SCI and chronic musculoskeletal shoulder pain. INTERVENTIONS: Participants were randomly assigned to receive 10 treatments of either acupuncture or handicap scooters, invasive sham acupuncture (light needling of nonacupuncture points). MAIN OUTCOME MEASURE: Changes in shoulder pain intensity were measured using the power wheelchairs User’s Shoulder Pain Index. RESULTS: Shoulder pain decreased significantly over time in both the acupuncture and the sham acupuncture groups (P=.005), with decreases of 66% and 43%, respectively. There was no significant difference between the 2 groups (P=.364). There was, however, a medium effect size associated with the acupuncture treatment. CONCLUSIONS: There appears to be an analgesic effect or a powerful placebo effect associated with both acupuncture and sham acupuncture. There was a medium treatment effect associated with the acupuncture, which suggests that it may be superior to sham acupuncture. Individuals with multiple sclerosis (MS) report decreased satisfaction with their handicap scooters devices compared with individuals with spinal cord injuries (SCIs). This study (1) investigated the demographic differences between veterans with MS (V-MS) and veterans with SCI (V-SCI) who were issued a wheelchair by the Veterans Health Administration (VHA) and (2) described differences in mobility device prescription. We merged two VHA databases to obtain demographic and wheelchair information for all V-MS and V-SCI in 2000 and 2001. Descriptive information for issued wheelchairs was available for 2,154 V-MS and V-SCI. We found that V-MS were significantly less likely to receive higher quality wheelchairs (manual or power) compared with V-SCI (p < 0.001). The disparity in VHA medicare power wheelchairs prescription between these two groups indicates a need for further research regarding the assistive device prescription process in these populations.
