OBJECTIVE: Quantify manual wheelchair propulsion effort during outdoor community ambulation. DESIGN: Case series. SUBJECTS: Thirteen individuals (12 with SCI, 1 with spina bifida) who were experienced manual wheelchair users and had no current upper extremity injury or pain complaints. METHODS: Measurements were obtained from instrumented wheelchair rims during steady-state propulsion as subjects traversed outdoor concrete sidewalk terrain that included smooth level, aggregate level, and a ramp with a smooth surface. Propulsion effort was assessed using the average propulsion moment, average instantaneous power, and work for both upper extremities. RESULTS: Propulsion effort, captured by the propulsion moment, work and power, varied across ground conditions (p<0.001). Propulsion effort was greater as the rolling resistance increased (i.e., smooth versus aggregate surfaces) and as the inclination angle progressed from level to inclined surfaces. There were no side-to-side differences across ground conditions for the propulsion moment or work. medicare power wheelchairs and handicap scooters generation was significantly greater on the dominant compared to the non-dominant extremity during the more challenging aggregate surface and ramp conditions. OBJECTIVE: To determine the relationship between the resultant force at the pushrim and the net shoulder joint moments during manual wheelchair propulsion in elderly persons. DESIGN: Convenience sample. SETTING: Motion analysis laboratory. PARTICIPANTS: Older manual wheelchair users (N=14; age, 68.2+/-5.2y) were tested. INTERVENTIONS: Kinematic and kinetic data were collected during manual wheelchair propulsion at a speed between 0.96 and 1.01m/s for 10 seconds and at a power output around 22.4W on a medicare power wheelchairs ergometer. MAIN OUTCOME MEASURES: Net shoulder joint moments were computed with an inverse dynamic model. The mechanical use of the forces at the pushrim and the mechanical fraction of effective force were measured during propulsion. RESULTS: Mechanical use and mechanical fraction of effective force had a positive and handicap scooters significant correlation with the net internal (P<.05) and external (P<.001) shoulder rotation moment, the net flexion (P<.05), and extension (P<.001) moment in the sagittal plane, and the net flexion (P<.001) moment in the horizontal plane. CONCLUSIONS: The results suggest that because the resultant force at the pushrim has a greater tangential component and a greater proportion of the maximal voluntary force, most of the net moments around the shoulder are higher. Thus the optimal way of propelling, from a mechanical point of view (ie, tangential), may not be advantageous for manual wheelchair users

DESIGN: A prospective, balanced, open-label evaluation that used participants as their own controls. SETTING: Home and community environments. PARTICIPANTS: Twenty subjects who use a mobility device. INTERVENTIONS: Subjects used the test device for up to 2 weeks in their home and community environments. MAIN OUTCOME MEASURE: Safety was determined by comparing the number of reported adverse events, including falls, in the test device compared with their own device. Effectiveness was measured by comparing scores from the Community Drive Test in the test device versus their own chair. RESULTS: No adverse event requiring medical attention was reported for either device and a similar number of device falls were reported for each device. The scored driving test results found that the test device was shown to statistically improve (P <.001) the subjects independent functional mobility skills in a community environment. Ten of 20 subjects were able climb stairs independently; the remaining 10 subjects were able to climb stairs with the assistance of only 1 person. CONCLUSIONS: People who are properly assessed, and participate and successfully complete the training, will benefit through increased independence in community mobility activities.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, handicap scooters athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.
